RIDOH Outbreak Reporting System

This is the RIDOH Outbreak Reporting System for Respiratory and Gastrointestinal outbreaks at facilities in Rhode Island.

Step 1: Choose the type of facility you are reporting for, then click "Next Page".

Step 2: On the next page you will find your facility in the list (just start typing). If you cannot find your facility, click the link to "add your facility".

Step 3: Follow the prompts on the next pages to add outbreak details like number of residents and staff who are ill, observed symptoms, and testing results or vaccinations, if relevant.
