The HPLRP is a critical component of the State's efforts to increase access to primary care, dental, and behavioral health services and reduce health disparities among our most vulnerable populations. The program serves as an essential workforce tool for recruiting and retaining high-quality healthcare providers working with underserved populations statewide in identified Health Professional Shortage Areas (HPSAs).

Submission of a complete Site Eligibility Application to the Office of Primary Care and Rural Health indicates your sites interest in obtaining site eligibility for the HPLRP.
